Complete Medical Insurance

You will be asked to provide your insurance information at the time of registration. It is important that USA Fibroid Centers has your current, complete and accurate medical insurance, referral and/or authorization forms to reduce delays and potential out–of–pocket costs.

Co-Payments

Depending upon your insurance coverage, you may be asked for a portion of your co–pay or co–insurance amount for the services provided at the time of your appointment.

Billing Your Insurance

Our billing team submits all required claims directly to your insurance provider. If there are remaining patient-responsibility items—such as deductibles, copays, or coinsurance—these will be billed to you after your claim has been processed. Our goal is to make insurance billing seamless so you can focus on getting the care you need.

Patient Statements

If you are uninsured or your insurance company does not pay for your treatment in full, you will receive a patient statement sent by our team. There are times insurance plans do not cover the entire cost of your care, resulting in a “patient portion.” You are responsible for the portion of the bill that your insurance company does not pay. We will send you a statement after your insurance has paid to notify you of any remaining balance.
